After residential treatment
Treatment is the start. Home is the test.
Returning home brings familiar responsibilities, relationships, routines and pressures back into your life, often before new habits have had time to settle.
The Foundations Continuing Care Programme provides 12 weeks of structured support after residential treatment. It helps you carry the progress made in Bali into everyday life, with ongoing contact from a team that already understands your story.
What is included
Structure, connection and support wherever you live
The programme combines individual clinical support, group connection and a consistent daily rhythm, beginning from the time you return home.

Continuing care questions
Clear answers before you return home
We arrange the practical details with you before the programme begins.
How long is the Continuing Care Programme?
The core programme lasts for 12 weeks and includes structured reviews at 30, 60 and 90 days. Month-to-month continuation is available after week 12 for people who want the structure to continue.
How is continuing care delivered?
The programme is delivered online once you return home. Your personal schedule is agreed before the programme begins and arranged around your location and time zone.
Will I continue with the same team?
Where possible, you will continue working with clinicians and programme staff who already understand your history and the work completed during treatment.
Can my family be involved?
The programme includes one facilitated online session with a nominated family member or partner, with your consent. Additional family or couples sessions can also be arranged.
What happens if I lapse or feel close to relapse?
Contact the team as soon as possible. We can review what happened, arrange additional support, increase check-ins and update your relapse-prevention plan. Where a higher level of care is needed, we will help you understand the appropriate next step.
Is Continuing Care the same as the Transitions Programme?
Continuing Care provides structured online support while you are living at home. Transitions is an optional supported step-down programme in Bali for people who would benefit from more time before returning home.
